Lo Loestrin Fe is a prescription birth control pill used for the prevention of pregnancy. If you are moderately obese, discuss with your healthcare provider whether Lo Loestrin Fe is appropriate for you.

IMPORTANT RISK INFORMATION

Do not use Lo Loestrin Fe if you smoke cigarettes and are over 35 years old. Smoking increases your risk of serious cardiovascular side effects (heart and blood vessel problems) from birth control pills, including death from heart attack, blood clots, or stroke. This risk increases with age and the number of cigarettes you smoke.

About This Drug

What is Lo Loestrin Fe (Norgestrel/Ethinyl Estradiol)?

Lo Loestrin Fe (Norgestrel/Ethinyl Estradiol) contains a combination of female hormones that prevent ovulation (the release of an egg from an ovary). This medication also causes changes in your cervical mucus and uterine lining, making it harder for sperm to reach the uterus and harder for a fertilized egg to attach to the uterus.

Lo Loestrin Fe (Norgestrel/Ethinyl Estradiol) is used as contraception to prevent pregnancy.

Lo Loestrin Fe (Norgestrel/Ethinyl Estradiol) may also be used for other purposes not listed in this medication guide.

What is the most important information I should know about Lo Loestrin Fe (Norgestrel/Ethinyl Estradiol)?

Do not use birth control pills if you are pregnant or if you have recently had a baby. Do not use this medication if you have a history of stroke or blood clot, circulation problems (especially if caused by diabetes), a hormone-related cancer such as breast or uterine cancer, unusual vaginal bleeding, liver disease or liver cancer, severe high blood pressure, severe migraine headaches, a heart valve disorder, or a history of jaundice caused by birth control pills.

Some drugs can make birth control pills less effective, which may result in pregnancy. Tell your doctor about all medications you use.

What should I discuss with my healthcare provider before taking Lo Loestrin Fe (Norgestrel/Ethinyl Estradiol)?

This medication can cause birth defects. Do not use if you are pregnant. Tell your doctor right away if you become pregnant, or if you miss two menstrual periods in a row. If you have recently had a baby, wait at least 4 weeks before taking birth control pills (6 weeks if you are breast-feeding). Do not use this medication if you have:

a history of a stroke or blood clot;
circulation problems (especially if caused by diabetes);
a hormone-related cancer such as breast or uterine cancer;
unusual vaginal bleeding;
liver disease or liver cancer;
severe high blood pressure;
severe migraine headaches;
a heart valve disorder; or
a history of jaundice caused by birth control pills.

If you have any of these other conditions, you may need a dose adjustment or special tests:

high blood pressure, heart disease, congestive heart failure, angina (chest pain), or a history of heart attack;
high cholesterol or if you are overweight;
a history of depression;
gallbladder disease;
diabetes;
seizures or epilepsy;
a history of irregular menstrual cycles; or
a history of fibrocystic breast disease, lumps, nodules, or an abnormal mammogram.

The hormones in birth control pills can pass into breast milk and may harm a nursing baby. This medication may also slow breast milk production. You should not breast-feed while you are taking birth control pills.

How should I take Lo Loestrin Fe (Norgestrel/Ethinyl Estradiol)?

Take exactly as prescribed by your doctor. Do not take in larger or smaller amounts or for longer than recommended. Take your first pill on the first day of your period or on the first Sunday after your period begins (follow your doctor's instructions).

You may need to use back-up birth control, such as condoms or a spermicide, when you first start using this medication. Follow your doctor's instructions.

The 28-day birth control pack contains seven "reminder" pills to keep you on your regular cycle. Your period should begin while you are using these reminder pills.

You may have breakthrough bleeding, especially during the first 3 months. Tell your doctor if this bleeding continues or is very heavy.

Take one pill every day, no more than 24 hours apart. When the pills run out, start a new pack the following day. Get your prescription refilled before you run out of pills completely.

If you need medical tests or surgery, or if you will be on bed rest, you may need to stop using this medication for a short time. Any doctor or surgeon who treats you should know that you are using birth control pills.

Store at room temperature away from moisture and heat.

What happens if I miss a dose?

Missing a pill increases your risk of becoming pregnant.

If you miss one "active" pill, take two pills on the day that you remember. Then take one pill per day for the rest of the pack.

If you miss two "active" pills in a row in week one or two, take two pills per day for two days in a row. Then take one pill per day for the rest of the pack. Use back-up birth control for at least 7 days following the missed pills.

If you miss two "active" pills in a row in week three, or if you miss three pills in a row during any of the first 3 weeks, throw out the rest of the pack and start a new one the same day if you are a Day 1 starter. If you miss three "active" pills in a row during any of the first 3 weeks, throw out the rest of the pack and start a new pack on the same day if you are a Day 1 starter. What should I avoid while taking Lo Loestrin Fe (Norgestrel/Ethinyl Estradiol)?

Do not smoke while taking birth control pills, especially if you are older than 35. Smoking can increase your risk of blood clots, stroke, or heart attack caused by birth control pills.

Birth control pills will not protect you from sexually transmitted diseases--including HIV and AIDS. Using a condom is the only way to protect yourself from these diseases.

Get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat. Stop using this medication and call your doctor at once if you have any of these serious side effects:

sudden numbness or weakness, especially on one side of the body;
sudden headache, confusion, problems with vision, speech, or balance;
chest pain or heavy feeling, pain spreading to the arm or shoulder, nausea, sweating, general ill feeling;
a change in the pattern or severity of migraine headaches;
nausea, stomach pain, low fever, loss of appetite, dark urine, clay-colored stools, jaundice (yellowing of the skin or eyes);
swelling in your hands, ankles, or feet;
a breast lump; or
symptoms of depression (sleep problems, weakness, mood changes).

Less serious side effects may include:

mild nausea, vomiting, bloating, stomach cramps;

freckles or darkening of facial skin;

Usual Adult Dose for Contraception:

Ethinyl estradiol-norgestrel products are packaged in 21 or 28 day dosage preparations. The last 7 tablets in 28 day dosage preparations are hormonally inert.

Regardless of the number of tablets in a package, the cycle length for oral contraceptives is generally considered to be 28 days. (The first day of menstrual bleeding is counted as day 1.)

Initiation of Oral Contraceptive Therapy

This product can be administered in two ways.

When initiating a Sunday start regimen, the first tablet may be taken on the first Sunday after menstruation begins. If a period begins on a Sunday, the first tablet may be taken on that day. When initiating a Sunday start regimen, another contraceptive method should be used until after the first 7 consecutive days of administration. For a 28 day package, one tablet is taken daily for 28 days and a new package begun on the following day. For a 21 day package, one tablet is taken daily for 21 days followed by 7 days with no medication. A new package of contraceptives is begun on the following day.

When initiating a Day 1 start regimen, the first tablet is taken on the first day on menstruation. Such initiation may increase the risk of spotting and breakthrough bleeding but decrease the risk of early ovulation and pregnancy. For a 28 day package, one tablet is taken daily for 28 days and a new package begun on the following day. Many clinicians recommend that additional contraceptive methods be used during the first cycle of hormonal therapy in order to reduce the risk of unintended pregnancy.

If a woman misses one dose of active tablets, the missed dose should be taken as soon as it is remembered and the normal schedule should be resumed.

If a woman misses two doses in week 1 or week 2 of the cycle, 2 tablets may be taken as soon as they are remembered and 2 tablets taken the next day and the normal schedule may be resumed. (Additional contraceptive methods should be used for 7 days.)

If a woman misses two doses in week 3 or three doses at any time in the cycle, Day 1 starters should discard the current package and begin a new package that same day. Sunday starters should take 1 tablet daily from the current package until Sunday, when the current package is discarded and a new package begun. (Additional contraceptive methods should be used until the woman has taken at least 7 days of hormonal therapy from the new package.)

Usual Adult Dose for Postcoital Contraception:

Ethinyl estradiol-norgestrel 50 mcg-500 mcg (Ovral), 2 tablets no later than 72 hours after having unprotected sex followed by 2 more tablets exactly 12 hours after the first dose.

Ethinyl estradiol-norgestrel 30 mcg-300 mcg (Lo/Ovral), 4 tablets no later than 72 hours after having unprotected sex followed by 4 more tablets exactly 12 hours after the first dose.

Patients should be advised to begin therapy at a time that it will make it convenient to take the second dose 12 hours later. Pretreatment with a long acting antinauseant such as meclizine will lessen the incidence of nausea.

modafinil (Provigil);
dexamethasone (Decadron, Hexadrol);
an antibiotic;
seizure medicines such as phenytoin (Dilantin), carbamazepine (Tegretol), oxcarbazepine (Trileptal), topiramate (Topamax), and others;
a barbiturate such as phenobarbital (Solfoton) and others; or
HIV medicines such as atazanavir (Reyataz), indinavir (Crixivan), saquinavir (Invirase), fosamprenavir (Lexiva), ritonavir (Norvir), and others.

This list is not complete and other drugs may interact with birth control pills. Tell your doctor about all medications you use. This includes prescription, over-the-counter, vitamin, and herbal products. Do not start a new medication without telling your doctor.

Possible side effects from taking Lo Loestrin Fe include nausea, spotting between periods, breast tenderness, headache, acne, decreased libido, bloating, blotchy skin, weight gain, higher blood sugar and cholesterol levels, and depression. What if I forget to take a dose of Lo Loestrin Fe?

If you miss one dose of Lo Loestrin Fe, take the missed dose as soon as you remember it. Then take your next dose at the regular time. You do not need back-up contraception in this case. If you miss two pills in a row in week 1 or 2, take two pills on the day you remember and two pills the next day to catch up. You will need to use back-up contraception for 7 days. Just got back from the Gyno. For some time now I've been miserable for 7+ days a month, when my period hits. October was the first month I experienced two long periods in one month, so I decided to go for a check up.

The doctor came to the conclusion that our first step to figuring out my menstrual issues was to put me on the pill. He prescribed Lo Loestrin Fe, which I'm supposed to start on Sunday. I got home and started Googling user reviews and side effects and I'm hesitant to try it.

Please share your experience with this, and any other alternatives there may be.

I hadn't been on birth control for about seven years, but needed to get back on it, so I tried the Paragard IUD. My body didn't like that so I tried the Mirena. Same thing -- it tried to expel it. So I went to the Lo Loestrin pill, and had to stop after two months. I knew the moment I needed to get off of it when I was crying, bawling, and called my SO and said "I don't know what I'm crying about!" I haven't been on anything since (SO had a vasectomy).

The reason I'm going on BC isn't for contraception, it's to control my periods since they're so heavy, long, and painful. I'd rather avoid BC as a whole, if possible. but I don't know what else we can try?

Personally, I would recommend drinking 1 Red Raspberry Tea bag from Yogi daily, that has really helped my periods be more regular and not as heavy. That has just been my personal experience :)

Hi! Thanks for your response. I am actually recovering from surgery right now! After I posted this thread I got really sick. I was crippled by the pain and nausea I was feeling. Had an ultrasound done and the tech found a cyst in my right ovary that was measuring 5cm. I was sent home and told to make a follow up appointment with my GYNO but the symptoms worsened and I ended up in the ER a few days later. They did another ultrasound there and found it had grown to be over two inches, and my doctor was called to set up surgery for the following day. It turns out the cyst was also putting pressure on my bowel (which was causing abdominal pain too).
I had never heard of tea helping regulate periods, but definitely sounds like it's worth a try. Thanks!

Lo Loestrin® Fe is an estrogen/progestin combination oral contraceptive (COC) indicated for use by women to prevent pregnancy. The efficacy of Lo Loestrin Fe in women with a body mass index (BMI) of >35 kg/m 2 has not been evaluated.

Warning: cigarette smoking and serious cardiovascular events Cigarette smoking increases the risk of serious cardiovascular events from combination oral contraceptive (COC) use. This risk increases with age, particularly in women over 35 years of age, and with the number of cigarettes smoked. For this reason, COCs should not be used by women who are over 35 years of age and smoke.

Lo Loestrin Fe is contraindicated in pregnant patients, in women with a high risk of arterial or venous thrombotic diseases, liver tumors (benign or malignant) or liver disease, undiagnosed abnormal uterine bleeding, or breast cancer or other estrogen- or progestin-sensitive cancer, now or in the past, and in women using Hepatitis C drugs containing ombitasvir/paritaprevir/ritonavir, with or without dasabuvir.

Warnings and Precautions

Discontinue Lo Loestrin Fe if a thrombotic event occurs, and at least 4 weeks before and through 2 weeks after major surgery. Lo Loestrin Fe should not be started any earlier than 4 weeks after delivery, in women who are not breastfeeding. If jaundice occurs, treatment should be discontinued.

Lo Loestrin Fe should not be prescribed for women with uncontrolled hypertension or hypertension with vascular disease. Women who are pre-diabetic or diabetic should be monitored while using Lo Loestrin Fe. Alternate contraceptive methods should be considered for women with uncontrolled dyslipidemia. Patients using Lo Loestrin Fe who have a significant change in headaches or irregular bleeding or amenorrhea should be evaluated.

In the clinical trial for Lo Loestrin Fe, serious adverse reactions included deep vein thrombosis, ovarian vein thrombosis, and cholecystitis. The most common adverse reactions (incidence ≥2%) were nausea/vomiting, headache, bleeding irregularities, dysmenorrhea, weight fluctuation, breast tenderness, acne, abdominal pain, anxiety, and depression.

Patients should be counseled that COCs do not protect against HIV infection (AIDS) and other sexually transmitted diseases.
